Our services are for anyone that needs a Level 2 Background Check for Florida Department of Health, Department of Children and Families (DCF), Agency for Health Care Administration (AHCA), Florida Department of Business Professional Regulation, Florida Department of Education, Florida Department of Highway Safety and Motor Vehicles, and Centers for Medicare and Medicaid Services.  See your Originating Agency Identifier (ORI) code below. 

be prepared to bring to the following to your Level 2 Background Check:

  1. A Valid Government Issued Photo ID (Driver's License, Passport, Work Visa, Military ID)

  2. Second Form of Identification (Credit Card, Bank Statement, Vehicle Registration, School ID, etc.)

  3. Your Originating Agency Identifier (ORI) code. This number identifies the agency requesting the criminal history check and for what purpose. See below for your specific ORI Code.
